How to Prepare for Your Appointment

    Fill out our new patient form(s), as completely and as detailed as possible. These forms can be partially completed through our patient portal.

  5. Contact your referring provider, if you have one, to make sure that they know you’ve been scheduled at our office so that they can send your medical records over.

    Write down any questions you may have for Dr. Sandford- some things can slip your mind once you sit down and get started with your appointment. Sometimes, it helps to bring a friend or family member with you to your appointment.

    Make sure to arrive 15-20 minutes before your scheduled appointment with your completed paperwork, photo ID and insurance card in hand.

How can I get a copy of my medical records?

We mail each patient with a copy of his or her visit note. Should you require any additional copies, or would like a copy of your complete record, just let our office know. You can also access your records through the patient portal.
